Please complete the form in full, attach a recent passport photo which shows a clear likeness to you, sign the form and return it to us by freepost; the freepost address for both our UK and Ireland shops is at the bottom of the form.

Alternatively, you can go to one of our shops and a Paddy Power staff member will support you through the self-exclusion process. Our shop colleagues can also provide you with a copy of our Safer Betting & Gaming leaflet, which contains information on support organisations that can help you if you need support with your gambling.
